Agency Partners are our lifeline to locating and addressing the individuals of the Erie County community.
If you are an agency partner, or would like to be, please complete the Agency Referral Form (either via the online form below, or download and print the PDF and return it to our organization.
Convalescent Fund Policy:
- Location: Erie County residents only.
- Frequency: One time annually.
- Grant Amount: $300-$500 depending on the need.
- Restriction: No reimbursements of any kind.
